The purpose of the program is to maintain persons who are severely and persistently mentally ill in the community, and to prevent long term psychiatric hospitalization. The ACTP team is responsible for identifying and assessing needs, treatment planning, coordinating services, and providing direct service to the consumers enrolled in the program. The team works closely with the agency psychiatrist in providing medication management as well as other medical related services. ACTP services are provided in a package of eight core services. These core services include assessment, person centered planning, individual therapy, group therapy, health services, case management, medication management, and crisis intervention. The ACTP team works assertively in the community. The program focuses on providing services that are innovative and individualized to meet the specific treatment needs of the consumers enrolled in the program. To reduce the stigma attached to being a mental health consumer the program adopted the name "Touchstone", and refers to consumers enrolled in the program as members. The treatment interventions take place in the member's home, the community at large, or other natural settings. The program is payee of Social Security and SSI benefits for members who need assistance with budgeting and money management. The ACTP Team works hard to maintain the consumers they serve in the least restrictive environment. |
